Pixelution is a free Pixel-by-Pixel Icon and Cursor editor plugin for Runtime Revolution 2.7 or higher.
Includes support for any icon size up to 48 x 48, alpha-channels, and instant cursor previewing.
Once you are done editing your icon/cursor, drop it back into your stack. The ability to Import and Export icons as a PNG and GIF is also supported.
